Prisma Health seeks a licensed Physical Therapist to plan and implement individualized home health treatment programs. You will collaborate with an interdisciplinary team to develop patient care plans and provide supervision to PT assistants, students, and CNAs involved in care.

This role requires advanced clinical and interpersonal skills and active participation in program development. Responsibilities include documenting assessments, planning care, and guiding patients and caregivers on

Qualifications

• Bachelor's Degree from an APTA Accredited Program.
• 1 year of Physical Therapy experience.
• PHYSICAL THERAPIST-Licensed or eligible for licensure in the State of South Carolina (Lawson Code CLPT).
• Valid South Carolina driver's license (Lawson Code LSCD).
• In lieu of the above minimum requirements, be a graduate of a PT program approved by the SC Board.

Responsabilités

• Plans and implements treatment programs for patients in home health.
• Collaborates with an interdisciplinary team to develop individualized care plans.
• Supervises Physical Therapy Assistants, students, and CNAs in patient care.
• Documents assessments and plan of care timely using Agency terminology.
• Provides patient and caregiver education on diagnoses, progress, and outcomes.
