Responsibilities

• Performs appropriate initial physical therapy assessments and reassessments within CMS timeframes.
• Documents assessment findings, including OASIS assessment responses, accurately within CMS timeframes.
• Establishes the plan of care for the patient in collaboration with the physician and reviews referral documents.
• Sets realistic goals and expected functional outcomes for the patient.
• Designs a coordinated plan of intervention with other services the patient is receiving.
• Documents the plan of care timely using Agency terminology and abbreviations.
