Job summary

Prisma Health, Home Health Midlands, is seeking a licensed Physical Therapist to plan and implement patient-specific treatment programs and collaborate with an interdisciplinary team. You will supervise Physical Therapy Assistants, students, and CNAs, while supporting staff development and delivering high-quality patient care.

Responsibilities include initial assessments, outcome measurements, referrals when needed, and planning of care with physicians.

Qualifications

• Bachelor’s degree in Physical Therapy from a CAPTE-accredited program.
• Active State PT licensure and CPR certification are required.

Responsibilities

• Plans and implements specific treatment programs for individual patients according to physical therapy principles.
• Collaborates with an interdisciplinary team to develop individualized care plans.
• Provides supervision to Physical Therapy Assistants, students, and CNAs.
• Documents plan of care, assessments, and outcomes timely and accurately.
• Reviews referrals and collaborates with physicians to maximize patient outcomes.
• Establishes measurable short- and long-term goals within patient care episodes.
• Discharges patients when appropriate and coordinates follow-up care.
