Qualifications

• Active Physical Therapist license in PA.
• Minimum 1 year of PT experience.
• Valid driver’s license and ability to drive to patient homes.

Responsibilities

• Provide skilled, home-based physical therapy.
• Evaluate patients and create individualized plans.
• Educate patients and caregivers on exercises and safety.
• Maintain thorough EMR documentation.
• Travel to patient homes within territory.
• Collaborate with interdisciplinary team.
