Residential Home Health and Hospice in Lansdowne, PA is hiring a Physical Therapist for a home health position delivering skilled care to patients in their homes. Eligible for a $10,000 first-year success bonus for achieving key milestones.

You will evaluate patients, develop therapy plans, educate families, document in EMR, and travel to patient homes within your territory. A valid PT license and driving is required, with about 20% driving time.

Qualifications

• Active Physical Therapist license in the state you work.
• Minimum one year of experience as a Physical Therapist.
• Driver’s license and ability to travel ~20% of workday.

Responsibilities

• Deliver skilled, patient-centered home health care with an interdisciplinary team.
• Perform comprehensive evaluations and develop therapy plans to improve strength and mobility.
• Educate patients and caregivers on exercises and safe mobility strategies.
• Document thoroughly in EMR and ensure compliant records.
• Work independently while collaborating with the team.
• Travel to patient homes within the assigned territory.
