Job summary
21st Century Home Health Services (21HHS) is seeking a Physical Therapist Assistant to join our Vallejo-based team. Under a Physical Therapist, you will assist in delivering high-quality home health care to patients in their own homes to promote safety and independence.
The ideal candidate holds a California PTA license, CPR, a valid CA Driver's License, and is comfortable with electronic charting and devices.
Qualifications
- California PTA license required
- Home Health experience preferred
- CPR certification required
- Valid California Driver's License required
- Computer proficiency with electronic charting systems
- Strong organization and communication skills
- Ability to work independently and in a team
- Experience with interdisciplinary teams is highly desired
- Case management fundamentals helpful
- Leadership and empathy are valued traits
Responsibilities
- Provide PTA-supervised home health care to patients in their homes.
- Assist patients to maintain mobility and functional independence.
- Collaborate with interdisciplinary team to develop and implement care plans.
