21st Century Home Health Services (21HHS) is seeking a Physical Therapist Assistant to join our Vallejo-based team. Under a Physical Therapist, you will assist in delivering high-quality home health care to patients in their own homes to promote safety and independence.

The ideal candidate holds a California PTA license, CPR, a valid CA Driver's License, and is comfortable with electronic charting and devices.

Full Time Physical Therapist Assistant

Anticipated Territory: Vallejo

Under the supervision of a Physical Therapist, you will assist in providing appropriate aspects of quality patient care and address the needs of individuals in their homes, so they can live safely and productively in their home environment.

What you'll do as a 21st Century Home Health Physical Therapist Assistant:

• Examine each individual and develop a plan using treatment techniques to promote the ability to move, reduce pain, restore function, and prevent disability.
• Work with patients to prevent the loss of mobility before it occurs by developing fitness- and wellness-oriented programs for healthier and more active lifestyles.
