Job summary

21st Century Home Health Services, a leading home health agency, seeks a California-licensed Physical Therapist Assistant. You will visit patients in their homes, assess mobility, plan treatments, and implement wellness-focused programs to maintain independence.

Qualifications

• CA PTA license required.
• Home health experience preferred.
• CPR certification required.
• CA driver’s license.
• Excellent organization and communication skills.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Examine each patient and develop a plan using treatment techniques to promote movement, reduce pain, restore function, and prevent disability.
• Develop fitness- and wellness-oriented programs for healthier, more active lifestyles and prevent mobility loss.
