Job summary

The Periscope Group is seeking an independent Physical or Occupational Therapist to perform one-time, in-home medical necessity assessments in Lakeland, FL. You will assess, document, and provide expert recommendations to help members maintain independence.

As a 1099 consultant, you will travel to member homes, set your own schedule within assessment timeframes, and participate in our mentoring program to support professional growth.

Qualifications

• Must be currently licensed as a Physical or Occupational Therapist.
• Strong communication and problem-solving skills.
• Ability to lift up to 30 pounds and navigate varied home environments.
• Reliable transportation and ability to travel to member homes.
• Proficient with online portals and documentation on devices.

Responsibilities

• Perform one-time comprehensive in-home medical need assessments.
• Provide expert recommendations for equipment, services, and care to support independence.
• Complete documentation through secure online portal from any device.
