Signify Health, part of CVS Health, seeks a Per Diem Physician to provide in-home health evaluations for members in their homes, facilitating connections to the right care. You’ll review medications and medical history and perform a clinical evaluation while coordinating with the patient’s health plan.

In this role you will travel locally, conduct virtual visits as needed, and deliver diagnostic and preventive services.

Qualificações

• Active, unrestricted license(s) in coverage area(s).
• Multi-state licenses (preferred).
