Job summary

Amazing Care Home Health Services in Ogden is seeking a part-time Physical Therapist to provide home-based PT services. The PT will evaluate clients, develop and implement treatment plans, and collaborate with physicians and caregivers to ensure high-quality care in home and community settings.

Requirements include a Doctoral degree in PT, Utah PT license, active BLS, valid driver's license with auto insurance, and a preference for 1 year of PT experience. Mileage is provided for visits.

Qualifications

• Doctoral degree in physical therapy (PT) is required.
• Utah Physical Therapist license is required.
• Active BLS certification is required.

Responsibilities

• Provides compassionate and high-quality care that meets the needs of the client and family by performing evaluations and developing a plan of care with skilled interventions.
• Develops and implements physical therapy treatment plans in home and community settings.
• Coordinates care with referring physician and other health care providers to ensure safe and effective care.
• Instructs client and family on the treatment plan and verifies understanding.
• Maintains clinical competency in the theory and practice of physical therapy.
