Job summary

Amazing Care Home Health in Logan, Utah is seeking a Part-Time Physical Therapist to provide in-home PT services. You will implement treatment plans for clients of all ages in home and community settings, coordinating with physicians to ensure safe and effective care.

The role requires a doctoral PT, Utah license, and active BLS certification. A valid driver’s license and insurance are required; 1+ year of experience is preferred but not required. Mileage is provided for travel between visits.

Qualifications

• A doctoral degree in physical therapy (PT).
• Physical Therapist (PT) License in the state of Utah.
• Active BLS certification.
• Valid driver's license and auto insurance.
• Minimum 1 year of clinical PT experience preferred, not required.

Responsibilities

• Perform evaluations and develop a plan of care with skilled PT interventions.
• Develop and implement PT treatment plans in home and community settings.
• Coordinate care with physicians and other health care personnel.
• Instruct client and family in the treatment plan.
• Identify additional service needs and refer as appropriate.
• Prepare and submit documentation per policy.
• Maintain clinical competency in PT theory and practice.
