Job summary

Amazing Care Home Health is seeking a licensed Physical Therapist to join our Ogden team. You will deliver in-home physical therapy services to clients of all ages, implementing treatment plans and coordinating with physicians to ensure safe, effective care.

The role supports a flexible part-time schedule (10-20 visits per week) with mileage reimbursement between visits. A doctoral PT and Utah license are required, along with a valid driver''s license and BLS certification.

Qualifications

• Doctoral degree in Physical Therapy (PT) required.
• Active BLS certification required.
• Utah PT license required.
• Valid driver''s license and auto insurance often needed for mileage between visits.
• At least 1 year clinical PT experience preferred but not required.

Responsibilities

• Perform evaluations and develop plans of care with skilled interventions.
• Implement PT treatment plans in home and community settings.
• Coordinate care with physicians and other healthcare providers.
• Instruct client and family on treatment plans and home exercises.
• Identify additional service needs and refer as appropriate.
• Prepare and submit documentation per policy.
• Maintain clinical competency in PT practices.
