Job summary

The Easter Seals of Greater Houston seeks a full-time Physical Therapist to provide in-home services within the Conroe Early Childhood Intervention territory. You will evaluate and support infants and toddlers birth to 36 months with developmental delays or disabilities in their natural environments.

Responsibilities include assessing development, creating IFSP-based plans, coordinating with families and other professionals, and documenting progress.

Qualifications

• Bachelor's Degree in Physical Therapy required.
• Texas Physical Therapy License required.
• 3 years' experience, including infant/toddler population.
• Strong English communication skills.
• Ability to drive extensively in Conroe area.

Responsibilities

• Assess developmental skills of assigned clients from birth to 3 years old.
• Collect information from case history, parent report, observation of child, and other professionals.
• Interpret evaluation results, develop diagnostic impressions, and make recommendations.
• Implement intervention plans for children with developmental delays based on IFSP with 45-day writing requirement.
• Schedule and prioritize direct/indirect services and document contacts; provide services at home, daycare, or other environments.
• Manage scheduling to ensure minimum 65 hours of service monthly.
• Communicate effectively with families, physicians, and community members.
• Serve as Service Coordinator for assigned clients as outlined in the Service Coordination Module.
• Participate in development and delivery of educational materials and trainings.
• Identify and refer for related services as needed.
