Job summary

Giving Home Health Care is seeking a full-time Patient Coordinator to join our Case Management team. The role centers on coordinating patient care, advocating for patients during renewals, and ensuring clear communication between patients, families, and clinicians.

Ideal candidates have 1–3 years in a care team or patient navigation role, with MS Office proficiency and reliable transportation for local travel. A high school diploma is required, associate degree preferred.

Qualifications

• Minimum associate level education preferred.
• High school diploma or equivalent required.

Responsibilities

• Enhance the patient experience and ensure patients receive necessary support and resources.
• Accompany patients to benefit renewal appointments to advocate for the patient and program.
• Visit patient homes and update patient care.
• Communicate information to patients, caregivers, and physicians.
• Maintain HIPAA compliance and confidentiality.
• Demonstrate a warm, friendly attitude and strong work ethic.
• Manage time effectively and build strong relationships with patients and teams.
• Coordinate communication inside the office and with external partners.
