Job summary

Giving Home Health Care is seeking a full-time Patient Coordinator to join our Case Management team. You will coordinate patient care and ensure seamless communication between our healthcare team, patients and families to provide timely support and resources.

You will accompany patients to benefit renewal appointments, visit homes to update care plans, and uphold HIPAA guidelines with a warm, professional approach.

Qualifications

• Strong work ethic and relationship-building skills.
• Self-motivated with time-management abilities.
• Strong communication skills inside and outside the office.
• Experience with MS Office (Outlook, Word, Excel).
• Associate degree preferred; high school diploma or equivalent required.
• 1–3 years in a Care Team or Patient Navigation role.
• Dependable transportation to travel locally between locations.

Responsibilities

• Enhance patient experience and ensure patients receive necessary support and resources.
• Accompany patients to benefit renewal appointments and advocate for the patient and program.
• Visit patient homes and update patient care.
• Communicate information to patients, caregivers, and physicians.
