Job summary

Giving Home Health Care, LLC is seeking a full-time Patient Coordinator to join our Case Management team and support in-home care for patients with work-related health conditions. The role focuses on coordinating care, communicating with patients and physicians, and advocating for patients throughout treatment and benefit processes.

Ideal candidates will have strong communication and organizational skills, experience in care navigation or similar roles, and a commitment to compassionate,

Qualifications

• Strong work ethic and relationship building skills.
• Self-motivated with the ability to manage time appropriately.
• Strong communication skills inside of the office and outside.
• Experience with MS Office products (Outlook, Word, Excel, etc.).
• Minimum associate level education preferred. (High school diploma or equivalent is required.)
• Minimum 1-3 year of experience in a Care Team or Patient Navigation role.
• Dependable transportation to travel locally between locations.

Responsibilities

• Enhance the overall patient experience and ensure patients receive necessary support and resources.
• Accompany patients to benefit renewal appointments to advocate for the patient and the program.
• Visit patient homes and update patient care.
• Communicate information to patients, caregivers, and physicians.
• Understand HIPAA and maintain confidentiality
• Display a warm and friendly attitude
