Job summary

Giving Home Health Care, LLC in Westminster, CO is seeking a Patient Coordinator to support our healthcare team and patients. You will ensure clear communication among patients, caregivers, and physicians, coordinate home visits, and assist with care planning to improve overall patient experience.

Qualifications

Minimum 1–3 years in a Care Team or Patient Navigation role.
Healthcare experience preferred.
Strong empathy and communication skills are essential.

Responsibilities

Enhance patient experience and ensure access to necessary resources.
Accompany patients to benefit renewal appointments and advocate for them.
Visit patient homes and update care plans.
Communicate clearly with patients, caregivers, and physicians.
Maintain HIPAA confidentiality and professionalism.
Present a warm and friendly demeanor.
