Job summary

Family Practice Center, PC is seeking a compassionate Palliative Care Nurse Practitioner to join our Harrisburg office in a full-time role. The NP will conduct in-home assessments, manage symptoms, and lead advance care planning for adults with serious illness, traveling across the Harrisburg region and surrounding counties.

The role blends clinical work with program development, including creating referral pathways, documenting care, and educating staff.

Qualifications

• Master's or Doctoral degree from an accredited Nurse Practitioner program.
• Active, unrestricted PA RN license and CRNP licensure.
• National NP certification (AGPCNP, FNP, AGACNP, or equivalent).
• Current DEA registration and PA controlled substance prescriptive authority (or ability to obtain).
• 2–3 years of NP or advanced clinical experience with adults, preferably in primary care, geriatrics, hospice, or palliative care.

Responsibilities

• Conduct comprehensive in-home palliative assessments and manage symptoms.
• Lead advance care planning and goals-of-care discussions with patients and families.
• Coordinate care with PCPs, specialists, hospice and facilities; ensure smooth transitions.
• Develop workflows, referral criteria, and documentation standards for the palliative program.
