Qualifications

As a PRN Palliative Care Nurse Practitioner (NP) with Impact Health PC in Christiana, PA, you will provide compassionate, patient-centered Palliative Care to residents within skilled nursing facilities.

You will assess and manage symptoms associated with serious and complex illnesses, develop individualized treatment plans, support goals-of-care discussions, and collaborate with physicians, facility staff, patients, families, and interdisciplinary care teams.

Key Responsibilities

• Provide compassionate, person-centered Palliative Care using evidence-based practices.

Qualifications
