Optum Home & Community Care is seeking a licensed NP or PA to conduct in-home preventive assessments as part of the HouseCalls program. You will evaluate medical history, perform exams, and coordinate follow-up care for patients in the home setting.

The role offers flexible scheduling (20-40 hours/week) with no on-call, weekends, or holidays, enabling work-life balance while delivering high-quality home-based care across Oregon.

Qualifications

• Active NP or PA license must be unencumbered and able to practice
• Active national NP or PA certification or eligibility by start date
• Ability to perform physical assessment including lifting up to 30 lbs
• Reliable transportation to complete home visit assessments

Responsabilités

• Conduct in-home assessments including evaluation of medical history, review of systems, med reconciliation, vitals, and physical exam
• Perform point-of-care screenings as appropriate
• Identify diagnoses for care management and treatment planning
• Document and communicate findings to inform the member’s PCP of gaps in care
• Identify urgent/emergent situations and intervene appropriately
• Educate members on disease processes, medications, and adherence
• Coordinate follow-up care and care plans with the broader team
