Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant to join the HouseCalls team for home-based assessments in Benton & Linn County, Oregon. This clinician-led care organization delivers preventive care and care coordination to improve health outcomes for vulnerable populations.

Role emphasizes in-home visits, lack of on-call and weekends, flexible scheduling (20–40 hours/week), and opportunities to grow within Home and Community Care.

Qualifications

• Active NP or PA license required and unencumbered.
• Active national NP/PA certification or ability to obtain by start date.
• Ability to perform physical tasks including lifting up to 30 pounds.
• Reliable transportation to complete home visits.

Responsibilities

• Conduct in-home assessments including medical history review, vitals, and physical exam.
• Perform screenings and point-of-care tests as appropriate.
• Identify diagnoses for care management and active medical management.
• Document findings to inform the member's PCP of care gaps.
• Identify urgent situations and intervene appropriately.
• Educate members on disease processes, medications, and adherence.
