Job summary

Optum HouseCalls is seeking an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage members in Oregon. The role emphasizes education, care coordination with primary care providers, and addressing social determinants of health.

Flexible scheduling is offered with no on-call, weekends, or holidays. Active licensure and state-specific certification are required, with travel to patient homes as part of the daily workflow.

Qualifications

• Active NP or PA license in Oregon or ability to obtain by start date.
• Certification in Family, Adult or Gerontology primary care (ANCC/AANP) or NCCPA for PA.
• Experience or ability to perform in-home assessments and education.

Responsibilities

• Conduct comprehensive in-home health assessments for Medicare Advantage and other plans.
• Document diagnoses and plan for care management and treatment.
• Educate members on disease processes, medications and adherence.
• Coordinate with PCPs to close gaps in care and address social determinants of health.
• Identify urgent situations and intervene when necessary.
