Job summary

Monogram Health is hiring a Field Outreach Representative to visit patients in their homes, build trust, and guide them toward enrolling in the Monogram Health program.

The role rewards independence and strong communication, with training and support provided to succeed in a field-based setting. This position involves travel across the assigned territory and meeting measurable enrollment targets.

Qualifications

• High school diploma or GED required.
• Valid driver's license and reliable transportation.
• Willingness to travel throughout the territory daily.
• Excellent communication, relationship-building, organization and follow-up skills.
• Ability to work independently toward measurable goals.
• Nice to have: Bachelor's degree or field outreach experience.

Responsibilities

• Visit eligible patients in their homes and introduce them to the Monogram Health program.
• Build trust through face-to-face conversations and help patients enroll.
• Work toward daily expectations for visits, conversations, enrollments and performance goals.
• Route efficiently across assigned patients and conduct follow-up to support enrollment goals.
• Record activities in CRM and stay accountable to monthly goals.
