Job summary

TalentBurst is seeking a Physical Therapist to provide outpatient rehabilitation services in patients' homes or in independent living facilities. The PT will evaluate, treat, educate, and coordinate with the rehab team to promote independence and reduce pathology.

This contract-based role supports flexible scheduling in various home settings. The position requires a valid PT license, appropriate degrees, and strong communication skills.

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy; or Master's in PT-related field with Bachelor's in PT; or Bachelor's in PT with 5 years of PT experience.

Responsibilities

• Supervise PTAs, temporary licensees, students per state licensure.
• Adhere to treatment schedule and complete billing/documentation.
• Attend patient care, staffing conferences, and related meetings.
• Refer to other disciplines to address patient needs.
• Collaborate with rehabilitation team to select appropriate therapies.
• Adapt plan of care to patient needs accordingly.
• Promote safety and maintain equipment.
• Provide home and community mobility assessments and guidance.
