Job summary

Qualifications

• Licensed Physical Therapist with a Master’s degree or equivalent in Physical Therapy.
• Must hold a valid Physical Therapy license in the state(s) of practice or license eligibility.
• Excellent organizational and time management skills; strong verbal and written communication.

Responsibilities

• Provide outpatient rehabilitation services in home/independent living settings.
• Develop and implement individualized therapy plans; document and report clinical progress.
• Supervise PT Assistants and support staff as required; coordinate with rehab team.
