Job summary

TalentBurst seeks a licensed Physical Therapist to deliver outpatient rehabilitation in in-home settings for the older adult population in Rochester, NH. You will assess, plan, and implement therapy to restore independence, educate patients and caregivers, and coordinate with the rehab team.

The role emphasizes hands-on therapy, documentation, and collaboration with the care network, with a 13-week assignment starting 11/23/2026, onsite and supporting patient mobility in home and community

Qualifications

• Must have a valid Physical Therapy license in the state(s) of practice.
• Master's degree in Physical Therapy or equivalent, with licensure eligibility.
• Foreign degrees must be evaluated for US equivalence.
• Must have a valid driver's license or reliable transportation.

Responsibilities

• Supervise PTAs, temporary licensees, students and support personnel as allowed.
• Adhere to treatment schedules, complete clinical and billing documentation.
• Attend care conferences and team meetings.
• Makes referrals to other disciplines to meet patient needs.
• Assess patient progress and adapt plan of care accordingly.
• Ensure safety and maintain equipment; manage supplies.
