Job summary

Humana’s CenterWell Home Health is seeking an experienced Occupational Therapy Assistant to deliver therapy services in patients’ homes. You will work under the supervision of an Occupational Therapist to implement treatment plans and improve strength, mobility and independence.

The role emphasizes home-based rehab, coordination with the care team, and patient/family education. Ideal candidates will hold OTA licensure with CPR certification, a valid driver's license, and access to reliable

Qualifications

Current OTA licensure and CPR certification required.
Six months OTA experience preferred for this role.
Home Health experience preferred to support patient goals and independence.

Responsibilities

Provide therapy services planned, delegated and supervised by the Occupational Therapist in accordance with the patient’s Plan of Treatment.
Assist in implementing vocational/education programs and activities established by the OT to restore and enhance function.
Design/adapt equipment and environment; fabricate devices to assist and improve function and independence.
Provide therapeutic treatment and instruction as directed to improve strength, coordination, ROM and function.
Document observations in patient records and assist with progress reports and required documentation.
Instruct patients and family on home programs and use of adaptive equipment.
Coordinate care and discharge planning and serve as a resource to other health care personnel.
Design community reintegration activities to assist physical reconditioning and adjustment.
