Job summary

Humana's CenterWell Home Health is seeking a full-time Occupational Therapist Assistant to support patients in South Kane and Dekalb Counties. You will deliver therapist-planned care, assist with program implementation, and adapt equipment to restore independence in patients’ homes.

You will work under an OT, coordinate with the team, and educate families on home programs. The role requires OTA licensure, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

Current and unrestricted OTA licensure.
Current CPR certification.
A valid driver's license, auto insurance, and reliable transportation.

Responsibilities

Provide therapy services planned, delegated and supervised by the qualified Occupational Therapist in accordance with the patient's Plan of Treatment.
Assist in implementing vocational/education programs and activities established by the OT.
Design/adapt equipment and living environments to improve function and independence.
Fabricate devices to assist and improve function as needed.
Provide therapeutic treatment and instruction to improve strength, ROM, and function.
Report observations to OT or Clinical supervisor and document in patient records.
Instruct patients and families on home programs and use of adaptive equipment.
Participate in care coordination and discharge planning with the interdisciplinary team.
