Job summary

CenterWell Home Health is seeking a licensed Occupational Therapist to support patients in their homes. You will assess daily living skills, design retraining programs, and work with physicians to implement the Plan of Care.

You will supervise staff, document care, and participate in discharge planning to help patients regain strength and independence. A driver’s license and reliable transportation are required.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable objectives.
• Confer with the patient's physician and care team to develop/revise the Plan of Care.
• Provide therapeutic treatment to improve/restore strength, coordination, ROM and function.
• Review/revise retraining programs to align with patient goals and rehab program guidelines.
• Document patient progress and ensure orders are recorded and communicated to the team.
• Participate in care coordination and discharge planning and liaison with family.
