Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one care in patients' homes or senior living communities in Onancock, VA. You will design personalized treatment plans, perform evaluations, and document in FOX’s EMR while collaborating with an interdisciplinary team.

The role offers autonomy, flexible scheduling, and opportunities to integrate telehealth and virtual care to support aging in place for older adults, with a strong emphasis on meaningful patient

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults wherever they call home (private residences, senior living communities, independent living, assisted living, memory care).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth/virtual care when appropriate to extend access and flexibility.
