Job summary

FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one‑on‑one care to older adults in Charlottesville, VA, in private residences or Senior Living Communities. You will create individualized plans of care and provide evidence‑based treatment, aiming to improve mobility, independence, and quality of life.

Qualifications

• Must hold an OT license in state(s) of practice or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency required.
• Current CPR certification is required.

Responsibilities

• Provide one‑on‑one occupational therapy to older adults in home or senior living settings.
• Perform evaluations, develop care plans, and deliver evidence‑based treatment.
• Deliver therapy sessions to improve mobility, independence, and safety.
• Document patient care in FOX’s EMR system.
• Collaborate with an interdisciplinary team of clinicians.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth or virtual care when appropriate to extend access.
