Job summary

FOX Rehabilitation is seeking a qualified Occupational Therapist to provide one-on-one in-home and senior living care across diverse settings. You will evaluate patients, develop plans of care, deliver evidence-based treatments, and document in FOX’s EMR system per regulatory standards.

Qualifications

• Active Occupational Therapy license in state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Proficient with EMR and basic computer skills; CPR certified.

Responsibilities

• Provide one-on-one OT to older adults in home and community settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Deliver telehealth/virtual care when appropriate.
