Job summary

FOX Rehabilitation is growing in Marion, IN and seeks licensed Occupational Therapists to deliver one-on-one care to older adults in homes or senior living communities. You’ll assess mobility, independence, and quality of life through our Geriatric House Calls model, with autonomy to schedule your appointments and collaborate with a multidisciplinary team.

This role supports telehealth options and in-person visits, offering a meaningful, flexible practice.

Qualifications

• Must hold a valid OT license in practicing state.
• Graduate from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
