FOX Rehabilitation is seeking licensed Occupational Therapists to deliver one-on-one care in homes and senior living communities in Lafayette, IN. You will assess patients, plan individualized care, and provide evidence-based therapy to improve mobility and independence.

Qualifikationen

• Valid Occupational Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency; CPR certification is current.

Aufgaben

• Provide one-on-one occupational therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that promote wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to enhance safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
