Job summary

Fox Rehabilitation in Carmel, IN is seeking licensed Occupational Therapists to deliver one-on-one care in private residences and senior living communities. You’ll build relationships, evaluate patients, and create plans of care that improve mobility and independence, with the option to integrate telehealth for flexibility.

The role offers autonomy, a flexible schedule, and collaboration with an interdisciplinary team to support aging in place and meaningful patient outcomes.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
