Fox Rehabilitation in Indianapolis, IN seeks licensed Occupational Therapists to deliver one-on-one geriatric home-based care within private residences and senior living communities. You’ll assess, plan, and provide evidence-based OT services with the option to incorporate telehealth for greater flexibility and continuity of care.

Formación

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsabilidades

• Provide one-on-one Occupational therapy to older adults in homes or living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care to extend access when appropriate.
