Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to join our in-home, senior living, and telehealth care team in Carmel, IN. You will deliver one-on-one OT services in private residences and communities, build patient-focused plans, and collaborate with PTs, SLPs, and Fitness Specialists to optimize mobility and independence for older adults.

Our Geriatric House Calls model supports flexible scheduling, autonomy, and meaningful outcomes.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, independence, and mobility.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
