Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist to deliver one-on-one geriatric care in Avon, IN, including private homes and senior living communities. The Geriatric House Calls model offers autonomy, flexible scheduling, and meaningful patient outcomes.

Qualifications

• Valid Occupational Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Occupational Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one occupational therapy to older adults in homes and communities.
• Perform evaluations, develop individualized care plans, and deliver evidence-based treatment.
• Deliver sessions to support mobility, independence, and safety.
• Document in FOX’s EMR according to regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
