JAG Physical Therapy in New Jersey is seeking a Per Diem Physical Therapist to join our outpatient orthopedic team. The role emphasizes evidence-based evaluation and treatment with precise data recording.

A valid New Jersey PT license, CPR certification, CEU compliance, and EMR proficiency are required; Doctorate is preferred. You will collaborate with a multidisciplinary care team and demonstrate strong communication and teamwork.

Qualifications

• Bachelor's or Master's or Doctorate; licensed PT in state.
• Licensed Physical Therapist in the state practicing in NJ.
• Completed CEU requirement in NY/NJ as applicable.
• Valid CPR License
• Experience with EMR software preferred

Responsibilities

• Collects and records patient care data following guidelines and reimbursement rules.
• Performs initial evaluation, daily notes, progress reports, discharge plans and team communications.
• Records daily billing and follows productivity guidelines.
• Prepares for evaluation per department policies and standards of practice.
• Selects and administers tests and measures.
• Synthesizes evaluation results to inform diagnosis, goals, prognosis and plan of care.
• Establishes treatment plan and collaborates with interdisciplinary team and patient support system.
• Identifies need for consultations and initiates discharge planning.
• Provides education/training to patient and support system.
• Functional training for ADLs/IADLs and rehab equipment training.
• Administers treatment and exercise prescription for caseload.
• Delivers safe, effective interventions in line with JAG standards.
• Documents notes using EMR; attends billing and coding seminars annually.
• Maintains respectful interactions with patients and colleagues.
