Job summary

CenterWell Home Health is seeking a dedicated Occupational Therapist Assistant to support patients in their homes under the supervision of a licensed Occupational Therapist. You'll contribute to rehabilitation plans, adapt activities for safety and independence, and educate patients and families on home exercise programs while collaborating with an interdisciplinary team to improve quality of life.

Qualifications

• Current OTA licensure and active CPR certification.
• A valid driver's license with auto insurance and reliable transport.
• Home Health experience preferred.

Responsibilities

• Provide therapy services planned and supervised by the Occupational Therapist.
• Assist in vocational/education programs to restore function.
• Design/adapt equipment and living environments to improve independence.
• Provide therapeutic treatment to improve strength, ROM and function.
• Document progress and coordinate with therapists and supervisors.
• Instruct patients and families on home programs and equipment use.
• Support care coordination and discharge planning with the team.
• Plan community reintegration activities to aid reconditioning.
