Qualifications

• Must have a valid OT license in the state(s) of practice.
• Must have a valid driver’s license or reliable transportation to patient homes.

Responsibilities

• Provide outpatient rehabilitation services in patients' homes.
• Implement OT treatment plans under supervision to increase independence.
• Maintain accurate and timely documentation for all patients.
• Collaborate with rehab team to develop and adjust patient programs.
• Ensure high-quality, compassionate care to patients and families.
