Job summary

Powerback is seeking a dedicated Occupational Therapist to provide in-home outpatient rehabilitation for older adults, delivering care in private residences or independent living facilities. The role is a full-time salaried position with a flexible schedule across the region.

The OT will conduct comprehensive assessments, design and execute therapy plans, and collaborate with a multidisciplinary team to maximize functional outcomes.

Qualifications

• Bachelor’s degree in OT from ACOTE-accredited program.
• Initial NBOT registration (and state licensure).
• Valid driver’s license or reliable transportation to patient residences.

Responsibilities

• Provide in-home outpatient OT services to older adults in private homes or IL/AL facilities.
• Develop and implement OT treatment plans to restore, improve and maintain function.
• Supervise and collaborate with therapy assistants and students as required.
• Maintain timely documentation of evaluation, treatment plans and progress.
• Coordinate with physicians and care teams to optimize patient outcomes.
• Ensure safety and infection control in home environments.
