Hireology is seeking a Home Care Occupational Therapist to provide skilled, patient-centered OT services in patients' homes across Michigan. You will evaluate needs, develop treatment plans, and deliver interventions to enhance independence in daily activities within home settings.

You will travel to patient homes, collaborate with a care team, document progress, and educate families on safety and disease management while complying with state regulations and agency policies.

Qualificações

• Must hold an OT degree from an accredited program.
• Active OT license in the practicing state.
• NBCOT certification as required by state regulations.

Responsabilidades

• Perform comprehensive in-home OT evaluations.
• Create individualized treatment plans based on assessments and goals.
• Deliver skilled interventions to improve daily living activities.
• Assess home environments for safety and recommend modifications.
• Educate patients and caregivers on adaptive techniques and energy conservation.
• Recommend and educate on adaptive equipment and devices.
• Document evaluations, treatments, progress, and discharge summaries.
• Collaborate with physicians, nurses, and other care team members.
• Reassess progress and adjust plans as needed.
• Provide fall and injury prevention guidance to patients and families.
