Optimal Care, Inc. is seeking a licensed Occupational Therapist to provide one-on-one home health services across Michigan. You will assess patients in their homes, develop personalized treatment plans, and guide independence in daily living activities.

You will collaborate with families and an interdisciplinary team, delivering education and hands-on training to help patients achieve meaningful, safe progress in their daily lives.

Formación

• Graduate of ACOTE-accredited occupational therapy program.
• Current state OT license.
• Minimum 1 year of experience as a licensed OT.
• Reliable transportation and daily travel to home visits.

Responsabilidades

• Assess functional abilities in patients' homes and design personalized treatment plans.
• Provide education, hands-on training, and encouragement to patients and families.
• Collaborate with interdisciplinary care team to monitor progress and adjust plans.
• Document care plans and patient outcomes for quality and compliance.
