Job summary

Argan Home Care in Port Charlotte, FL is seeking a compassionate Occupational Therapist to join our growing home-based team. You will evaluate patients at home, develop individualized treatment plans, and help patients regain independence to improve daily living.

Work closely with physicians and caregivers, provide ADL interventions, educate families, document progress, and ensure compliance with Medicare guidelines and documentation standards. A Florida OT license and NBCOT are required.

Qualifications

• Valid Florida OT license in good standing.
• NBCOT certification required.
• Graduate of an accredited OT program (Master's or Doctoral preferred).
• Home health experience and knowledge of Medicare guidelines preferred.
• Reliable transportation and valid driver's license for home visits in Port Charlotte area.
• Excellent communication and patient-centered care skills.

Responsibilities

• Conduct comprehensive occupational therapy evaluations for home care patients.
• Develop and implement individualized treatment plans to improve functional independence.
• Provide interventions targeting ADLs, fine motor skills, cognition, and home safety.
• Collaborate with physicians, nurses, and care team to coordinate patient care.
• Educate patients and caregivers on adaptive techniques and home modifications.
• Monitor progress and adjust plans as needed.
• Maintain accurate clinical documentation in compliance with standards.
