Job summary

Elite Home Health is seeking a dedicated Occupational Therapist to provide skilled OT services in patients' homes across Florida, following physician orders and the plan of care. You will evaluate functional status, develop interventions, and collaborate with the care team to enhance independence and safety.

Responsibilities include patient and family education, documentation, and ensuring compliance with Medicare requirements.

Qualifications

• Graduate of an accredited Occupational Therapy program required.
• Current state OT license in good standing required.
• Minimum of one year of OT experience; home health, acute care, or rehab preferred.
• Current CPR certification required.
• Valid driver’s license + reliable transportation.
• Able to pass background screening and compliance requirements.

Responsibilities

• Evaluate patient functional status and develop OT treatment plans.
• Provide skilled OT services in the home per physician orders and plan of care.
• Educate patients and families on therapy goals, safety, and home adaptations.
• Complete timely documentation and ensure compliance with Medicare/state regulations.
