Job summary

CenterWell Home Health in Florida is seeking a licensed Occupational Therapist to deliver in-home rehabilitation, helping patients regain strength, mobility and independence through personalized retraining programs.

You will collaborate with physicians and the care team, develop the Plan of Care, supervise aides, document progress and participate in discharge planning. A valid OT license, CPR, and driver's license are required.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver’s license, auto insurance and reliable transportation.

Responsibilities

• Assess patient daily living and develop retraining programs with measurable objectives.
• Confer with physicians and care team to develop the Plan of Care.
• Provide therapeutic treatment to improve strength, coordination, ROM and function.
• Document patient care observations and update treatment plans and discharge summaries.
• Supervise therapy staff and home health aides in OT service delivery.
• Coordinate care and participate in discharge planning.
