Mercy is seeking an Occupational Therapist to provide in-home therapy for patients with neuromuscular, skeletal and perceptual disorders, under physician direction. You will assess, plan and implement individualized treatment programs in patients' homes.

Ideal candidates have a valid state license, CPR certification and at least one year of experience in acute, rehab, outpatient, home health or long-term care settings, plus strong communication and documentation skills and the ability to lift up

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current state license; valid driver’s license.
• Minimum of one year of experience in acute, rehab, outpatient, home health or long-term care setting.
• CPR certification.
• Excellent assessment and treatment planning skills.
• Good communication and documentation skills.
• Ability to lift up to 50 pounds frequently; able to adapt to emergencies.

Responsabilités

• Provide in-home occupational therapy to patients with neuromuscular, skeletal and perceptual disorders under physician direction.
• Schedule visits and coordinate care with patients, physicians and office staff.
• Develop and implement individualized treatment plans and assess progress.
• Adapt to changing conditions, manage emergencies, and maintain confidentiality.
• Document care accurately and communicate findings to the care team.
