Job summary

Midland Care Connection is hiring an Occupational Therapist to provide home-based skilled services in the Topeka area. You will evaluate patients, develop personalized treatment plans, and collaborate with an interdisciplinary team to support recovery and independence.

Responsibilities include teaching patients and families, ensuring compliant documentation, and coordinating care with physicians and other providers. Travel to patients' homes is required.

Qualifications

• Graduate of an accredited Occupational Therapy program and KS license.
• Current CPR/BLS certification.
• Ability to travel throughout the Topeka area and work with an interdisciplinary team.

Responsibilities

• Evaluate patients in home settings and develop personalized treatment plans.
• Coordinate with physicians, nurses, therapists, and social workers to implement plans.
• Educate patients and caregivers and document progress in accordance with Medicare standards.
